1. I grant Living Word Christian Center and its related ministry entities (Kingdom Running Club) permission for the use of my image, voice and/or likeness to be captured and used on promos, website/internet and social media.

2. Living Word Christian Center may use my name, voice, picture, materials, and/or statements made by me during the running club season for promotional and ministry use including, but not limited to, websites, internet, and social media, or any other means of disseminating information relating to the running club.

3. Living Word Christian Center may edit my appearance as necessary and I hereby release ownership and waive my rights for participation in pictures, videos and/or audio footage.

4. I hereby indemnify Living Word Christian Center, its subsidiaries, agents or any representative thereof, against any claims arising out of my appearance on any Kingdom Running Club materials.

5. I hereby release Living Word Christian Center from any loss, claims or injuries I may incur arising out of my participation or appearance or its permitted uses.

6. *** If under the age of 18 you must have a parent or guardian sign the release authorizing the use of your image and/or likeness.

I undersigned certifies that the use of their photos or videos are solely voluntary. The undersigned agrees to allow Living Word Christian Center to use their likeness and image as necessary, desired and/or is mandated by Living Word Christian Center. I have carefully read the terms of this "release" and indicate my understanding and consent by signing below.

This agreement is between the participant (noted above) and Kingdom Running Club (hereinafter KRC).

1. I understand and agree that I am at least 18 years and have been fully advised of the risks involved with participation of beginning and being part of KRC as an organization that teaches and supports new and advanced runners for preparation to run and complete marathons.

2. I understand that the nature of running with the group and on my own is that running is physical and I agree that I am in good physical and mental health with no pre-existing medical conditions that would prevent me from participating in all of the aspects therein.

3. I understand and represent that I have been advised by KRC to consult with my physician as a Runner participant in the KRC Program and that I have actually and in fact consulted and obtained a physician’s recommendation that I may participate within the Kingdom Running Club (KRC) program.

4. I agree that I will hold KRC and all of its coaches, personnel, staff, volunteers and speakers harmless as to my participation in the KRC Running program.
5. I understand and agree that any instructions given to me by KRC are not for me individually, but are meant for the entire group of participants.

6. I understand that KRC will not cover any medical expenses resulting in injury incurred as part of my participation in the Program. I understand that there is no medical insurance coverage as part of my participation.

7. I understand and agree that my being permitted to participate in the KRC Program is adequate consideration for the releases, waivers, and indemnities given herein.

8. I hereby, on behalf of myself, my heirs, executors and administrators waive and release any and all rights and claims against the following entities and persons, and agree to indemnify and hold harmless the following entities and persons: Bill Winston, Living Word Christian Center, KRC and all of its coaches, personnel, staff, volunteers, speakers and their respective companies, any other entities involved with the KRC Program, their officers, directors, agents, employees and volunteers, with respect to any and all liability of any nature in any way arising out of the KRC Program and my participation therein, without limitation.

The foregoing release of liability includes all liability with respect to my travel to or from the KRC Program, wherever the program will meet.
